Profile

The National Health Laboratory Service (NHLS) is the public sector laboratory provider in South Africa, delivering a broad range of diagnostic tests that include pathology, microbiology, virology, chemistry and haematology for patients in public hospitals and clinics. Its services support clinical decision‑making, disease surveillance and outbreak investigation, and it also offers reference and specialised testing that is not routinely available elsewhere. The organisation operates a nationwide network of laboratories that process clinical specimens and generate results for clinicians and public health programmes. In addition to routine diagnostics, NHLS contributes to training of medical laboratory scientists and technologists.

As the national public health laboratory service, NHLS holds a statutory mandate to ensure the quality and safety of laboratory diagnostics across the country, overseeing proficiency testing and accreditation programmes for both public and private labs. It houses specialised units such as the National Institute for Communicable Diseases, which leads surveillance and research on infectious threats including HIV, tuberculosis and COVID‑19. The organisation’s expertise in large‑scale screening programmes has been highlighted during national health emergencies, where it rapidly expanded testing capacity to meet demand. Its role extends beyond patient care to informing public health policy through the provision of timely laboratory data.

NHLS functions as a statutory body under the National Department of Health, reporting to the Minister of Health and receiving government funding to fulfil its public health mandate. While it is not a privately owned company, it maintains operational autonomy in managing its laboratory network and service delivery. The organisation’s structure includes various divisions and specialised centres that focus on distinct disciplines such as anatomical pathology, chemical pathology and genetics. This governmental positioning distinguishes NHLS from commercial laboratory providers and aligns its objectives with national health priorities.

In June 2024, the NHLS suffered a ransomware attack that encrypted parts of its primary and backup servers, disrupting internal and external access to laboratory information systems. The attack did not result in the loss or unauthorized access of patient data, according to the organisation’s statement. In response, the NHLS activated its incident response team, enlisted internal and external cybersecurity experts, and isolated affected systems to prevent further spread. While automated report generation was interrupted, laboratories continued to process samples and convey urgent results to clinicians via telephone, and the organisation reported that it was rebuilding compromised systems with additional security layers in place.
